What to do if you want your old notes

You have the right to access your clinical record but this is done through a request process so we can make sure we release the right information to the right person.

Belverdale Hospital Ltd wishes to inform patients that all clinical records older than 10 years will be destroyed unless collected by 1 August 2022.
Under the Health Regulations 1996 (Retention of Health Information), Belverdale Hospital must keep any health records they hold for a patient for 10 years from the last time they provided services to that patient i.e. if you have not been a patient since 2011 then your records will be destroyed.

You must complete a ‘Release of Information' form and provide suitable identification before any information is released to you.
